This position will provide case management, assessment, one-on-one nursing care, crisis intervention (both by telephone and in person), education, and monitoring of patients receiving pharmacological treatment for a mental health diagnosis in a variety of inpatient and outpatient settings.  This position will also triage mental health and chemical health patients to ensure adequate and appropriate care and will serve as a resource and liaison for providers, members and the community.<br><br>Additional responsibilities include:<br><br>1. Promote patient optimum level of functioning:<br><br>&bull; Provide initial and ongoing assessment and triage of patient&#8217;s mental health needs and mental health status.  Determine disposition that meets patient needs and coordinate care with other providers and appropriate parties.<br>&bull; Develop, implement, evaluate and revise treatment plans.<br>&bull; Provide medication management in collaboration with the psychiatrist.<br>&bull; Develop and facilitate psycho-educational programs and groups for patients and family members.<br><br>2. Collaborate with other caregivers:<br><br>&bull; Consult with the psychiatrist regarding patient needs and implement the psychiatrist&#8217;s written and verbal orders.<br>&bull; Consult with mental health and medical providers regarding patient needs.  Document and follow through with treatment plans as needed and agreed upon.<br>&bull; Participate in the development of patient standards of care, policies and procedures.<br>&bull; Participate in multidisciplinary case conferences.<br>&bull; Participate in task forces and meetings as needed.<br><br>3. Follow medical/legal guidelines, policies, and procedures as established by HealthPartners, state<br>and federal laws / statutes and the Nurse Practice Act.
