Job Duties:

3. Recognize the need for and perform routine office procedures and testing in the ambulatory care setting.
4. Assist patients to achieve basic health care goals.
5. Identify patient education needs, provide instruction and refer appropriately.
6. Document observations, actions and patient outcomes.

Schedule:
This is an on-call position and actual schedule and hours will vary depending on coverage needed. Must be flexible and available Monday ? Friday daytime hours.

Accountabilities:
1. Provide direct participant lifestyle/behavior change coaching on health behaviors in a group setting. Core areas of focus include: physical activity, tobacco cessation, stress management, weight management, nutrition, and other health-related areas of preventive counseling.
2. Topics include: Stress management, tobacco cessation, nutrition, exercise, 5K training, and weight management. Health coaches would be available to coach on all topics based on credentials and expertise.
3. Work collaboratively with JourneyWell Health Management Consultants to understand the customer.
4. Work on a contract basis per group course. Health coach must commit to the entire series of the group course. Each group course usually consists of three or five one-hour sessions.
5. Deliver a series of presentations. Presentations may be delivered onsite at the employer setting or by webinar.
6. Responsible for having all materials ready for the presentations, including the presentation and all handouts.
7. Make appropriate referrals to employers purchase JourneyWell services.
8. Facilitate referrals to health management programs or worksite programs.
9. Submit participant roster to HealthPartners after each session to record participation or record participant in computerized record-keeping system.
10. Submit or record group course pre and post program evaluations.
11. Perform other duties as required to accomplish HealthPartners and company goals, as assigned by the Group Health Coach Manager.

Required Qualifications:
1. Bachelor?s degree in health education, exercise physiology, health promotion, or other health-related field.
2. Two years of experience in health promotion or preventive counseling experience on health or wellness-related issues.
3. Previous experience in health coaching.

Preferred Qualifications:
1. Master?s degree in health education, exercise physiology, public health, or other health-related field.

? Consulting with employees to facilitate changes in organizational culture supportive of health improvement initiatives and identify appropriateness of various types of program plans and facilitate integration of health improvement into their organizations.
? Coordinating effective culturally impactful communication and reporting strategies

This position requires a Bachelor?s degree in health education/promotion or a health related field along with at least five years of experience working in a health coaching role performing intrinsic coaching. In addition, program development, implementation and evaluation experience is required along with knowledge of the principles, theories, and practices of a worksite health promotion program, risk reduction, disease self management. Demonstrated project management experience and customer interaction experience is necessary.
